01-FEBRUARY-2008  MUNA MUHAMED IBRAHEEM  42 YEARS  EXTRUDED DISC L4-5 WITH LEFT FORAMINAL COMPRESSION.

Anamnesis:

The patient came to the emergency of the Al-Shmaisani Hospital 01-February-2008 complaining of LBP with left sciatica for 1 month, with exacerbation the last week.

MRI of the lumbar spine performed 2 days ago showing an extruded disc L4-5 with left foraminal occlusion.

bullet

On examination: the patient is unable to walk or stand and SLRS was 5 degrees in the left with drop left foot and weak planterflexion that foot. Hypalgesia of the left L5 territory.

Left L4-5 hemiflavotomy with foraminotomy of left L5 root was performed. The extruded disc was removed lateral to the axilla and meticulous cleaning of the disc space of L4-5 was done from the left side.

Smooth postoperative recovery with improvement of the motor power of the left foot.

Comments

Foraminotomy of the involved root must be done in all disc surgeries to be sure that there is no remnant left under the axilla.

bullet

The disc space height was shallow which means a low expected recurrence rate in this case.
