PLD L5-S1 WITH LEFT DOWNWARD MIGRATION.

Anamnesis
The
patient came
to the clinic 31-August-2010 complaining of LBP
for 3 years with left sciatica. The left
sciatica intensified the last month.
MRI lumbar spine
performed 25-August-2010 showing extruded disc
L5-S1 with left foraminal occlusion
On examination: SLRS
was 20 degrees in the left with pain and weak
dorsi 3/5 and planterflexion 4/5 left foot.
There is hypalgesia and numbness both left L5
and S1 territories.
Left L5-S1
hemiflavotomy with left S1 foraminotomy. The
extruded disc was removed in one piece lateral
to the axilla. Left sided cleaning L5-S1 disc
space.
Routine closure of
the wound and smooth postoperative recovery with
improvement of the power of the left foot and
disappearance of left sciatica.
Comments
The estimated postoperative
recurrence rate in this case is around the
average - 7%, because the disc space is still not
shallow.
