YEARS WIDE-BASED EXTRUDED DISC C6-7 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis:
The patient came to the
clinic 29-May-2008 complaining of neck and left
shoulder pain with limitation of neck movement
for 13 days after falling down from the horse.
He is suffering also from
numbness of the index of the left hand.
MRI cervical spine performed
24-May-2008 showing small PCD C5-6 in the right
side and a huge PCD C6-7 with wide-based
extrusion with left foraminal occlusion.
On examination: the patient
has weak flexion and extension of the left hand
4/5 with gross weak left triceps muscle 3/4.
Using the image-intensifier
and anterior approach, discectomy of C6-7 was
performed and all the extruded disc material was
removed in several pieces. The posterior part of
annulus fibrosis was fractured and shifted
posteriorly. It was removed accordingly.
Smooth postoperative recovery
and normalization of the power of the left upper
limb.
Comments
The recurrence rate in
cervical disc surgery is zero.
Single simple level
discectomy is not in need for stabilization or
insertion of artificial devices, since the
joints of Leushko and the anatomical
compartments provide sufficient degree of
stability.
Not all PCDs require surgical
interference such as the C5-6 in this case.
