EXTRUDED DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis:
The patient came to the
clinic 22-April-2008 complaining of LBP with
right sciatica for two months. She was limping
with SLRS 70 degrees with pain in the right and
80 degrees in the left. She had weak dorsi and
planterflexion right foot 4/5.
MRI performed 28-April-2008
showed right extruded disc L5-S1 with root
compression.
Rigt L5-S1 hemiflavotomy with
right S1 root foraminotomy was performed and the
extruded disc was removed lateral to the
axilla and meticulous cleaning of the disc space
was achieved from the right.
Routine closure of the wound
with smooth postoperative recovery.
Considerable improvement of
the power of the right foot.
Comments
Even the extrusion, was not
big in size, but it was rubbery in consistency
and compressing the root directly. In this case
the conservative treatment will fail, for what
surgery was preferred.
The estimated recurrent rate
in this case is around 7%, because the disc
height still acceptable, despite the fact that
meticulous cleaning was achieved, but from only
one side.
