The patient came to Dr. Ali Al-Bayyati complaining of severe neck pain and right upper limb pain for 4 months with exacerbation of pain the last month, depriving him sleeping.

MRI cervical spine performed showing huge extrusion C5-6 right side with foraminal occlusion and small PCD C6-7 left side.

On examination: The patient has neck movement limitation and pain when looking to the right and all directions with weak grip right hand 4/5, extension the right hand 3/5 and right triceps 2/5. There is numbness and hypalgesia of the thumb and index finger right hand.

Discectomy of C5-6 was done until the dura was seen in the right side of the field and inspection of the exit foramen of right C5-6 ruled out any remnant. Intradiscal decompression of C6-7 was also performed to reduce extrusion of the C6=7 disc in the future.

Smooth recovery with prompt improvement of the power of the right upper limb.

The patient having gross neurologic deficit and agonizing pain. The major problem was the C5-6 disc extrusion.

The expected recurrence rate in this case is zero. The unco-vertebral joint of C5-6 was not violated in the right side so as to minimize the postoperative pain. The C6-7 disc was intradiscally decompressed to prevent future extrusion.
