YEARS EXTRUDED DISC L4-5 WITH LEFT FORAMINAL OCCLUSION.

Anamnesis:
The patient came to the
clinic one month ago with LBP for 2 months and
left sciatica.
On examination: the patient
is limping with exaggerated scoliotic stance and
almost drop left foot and analgesia of left L5
root territory and hypalgesia of left S1 root.
SLRS was 40 degrees in the left with pain.
MRI of lumbar spine performed
showing huge extruded disc L4-5 with left
foraminal occlusion and down ward migration.
Foraminotomy of left L5 root
was performed and the extruded disc was removed
in several pieces. Meticulous cleaning of L4-5
disc space was achieved from the left side.
Considering that the young age of the patient
and the still considerable height of the disc
space, PEEK Satellite nucleus replacement sphere
No 11 was inserted. Check image-intensifier
showed central location of the device.
Routine closure of the
wound and smooth
postoperative recovery
and the patient sent to
the ward.
Comments
The expected recurrence rate
in this case is more than 10%, for what PEEK
sphere was applied to reduce this figure to
around 2%.
