HUGE EXTRUDED DISC L5-S1 LEFT SIDE.
Anamnesis:
The patient came to the
clinic 03-May-2009 complaining of left sciatica
for one month with exacerbation of the sciatica
and LBP the last 4 days. He came with crutches.
On examination: the patient
is limping with exaggerated scoliotic stance
with positive cough sign and SLRS 30 degrees in
the left and weak dorsi and planterflexion left
foot 4/5.
MRI lumbar spine performed
04-May-2009 showing huge extruded disc L5-S1
with left up and downward migration.
Foraminotomy of left S1 root
with partial flavotomy was performed. The
extruded disc was shifting the S1 root laterally
and it was removed subaxillary in several huge
pieces. The disc space was shallow and cleaning
of the disc space of L5-S1 was performed from
the relatively wide defect in the annulus
fibrosis.
Smooth postoperative recovery
with normalization of the power of the left
foot.
Comments
The estimated recurrence rate
in this case is below 7% since the disc space is
shallow, but the defect in the annulus fibrosis
was wide.
