11-MAY-2003 AHMAD ALI AL-WALEED 34 YEARS
RIGHT LESSER WING MENINGIOMA WITH INVOLVEMENT OF THE RIGHT CAVERNOUS SINUS.
Anamnesis:
The patient came to the
clinic 04-May-2003 complaining of headache for 2
years with PGE. Visual disturbances of the left
eye since childhood.
MRI of the brain done
20-March-2003 showing huge right lesser wing
meningioma with involvement of the ICA and the
right cavernous sinus.
Using right pterional
approach, subtotal resection of the tumor was
performed, leaving tiny fragments attached to
the right ICA and the lateral wall of the
cavernous sinus was coagulated.
The dura was water-tightly
closed and routine closure of the wound.
Smooth postoperative
recovery.
Comments
The patient has meningioma,
which was invading the wall of the right
supraclinoid of ICA and invading the lateral
wall of the cavernous sinus. All the soft tissue
component of the tumor was removed, but it was
impossible to remove the tumourously involved
segment of the ICA.
This fact increase the rate
incidence of recurrence, but leaving these parts
avoid catastrophic sequelae, if radical
resection was attempted.
The patient escaped form
follow up, because he was living in Libya.
