WIDE-BASED EXTRUDED DISC L4-5 WITH SECONDARY CANAL STENOSIS.

Anamnesis:
The patient came to the
clinic 11-July-2009 complaining of LBP for one
month with agonizing right sciatica.
MRI lumbar spine
performed the same day
showing wide-based
extruded disc L4-5
causing secondary
segmental stenosis.
On examination: the
patient is limping with
scoliotic stance and
weak dorsi and
planterflexion right
foot. The patient is a
known diabetic.
Bilateral cleaning of
L4-5 disc space was
performed. Considering
that the disc space of
L4-5 was still high and
the defect in the
annulus fibrosis was
wide, it was mandatory
to insert the Satellite
PEEK nucleus replacement
No 16 after the last
trail of No 15 from the
left side. Check X-ray
showed acceptable
position and inspection
from the right side of
the attack sowed the
artificial device
centrally located.
Smooth postoperative recovery
with improvement of the power of the right foot.
Comments
The expected recurrence rate
in this case is high, for what the Satellite
PEEK nucleus replacement was applied to prevent
further future collapse and minimize the rate of
postoperative recurrence.
