YEARS EXTRUDED DISC L5-S1 WITH LEFT FORAMINAL COMPRESSION.

Anamnesis:
The patient
came to the clinic 06-December-2009 complaining
of LBP with left sciatica for more than three
months.
On examination: the patient
had exaggerated scoliotic stance and limping
with agonizing bilateral sciatica and weak
dorsiflexion and planterflexion left foot -4/5.
MRI of lumbar spine performed
07-December-2009 showing huge extrusion L5-S1
disc with left foraminal occlusion.
The ligamentum flavum was
detached from the left side at L5-S1 level and
reflected to the right. The left S1 root was
exposed and it was severely compressed by the
extrusion. Cleaning of the disc space and the
extrusion lateral to the axilla. After
meticulous cleaning of L5-S1, PEEK Satellite
nucleus replacement sphere No 12 was inserted.
Check image-intensifier was performed at all
stages of surgery.
Routine closure of the wound.
Comments
The estimated recurrence in
this case was over 15%, because the disc space
was still high and the defect in the annulus
fibrosis was wide.
Insertion of
PEEK sphere is mandatory
to decrease the recurrence to 2%.
