7-AUGUST-2009  HANEEN MAMDOUH AL-MASRY  52 YEARS  HUGE RECURRENT PLD L5-S1 RIGHT SIDE.

Anamnesis:

The patient was operated by for huge PLD L5-S1  with right downward migration 31-May-2008. She was doing well until 1 month ago got right sciatica with exacerbation the last 10 days.

The patient came to the clinic 15-August-2009 and in clinical evaluation, she had no neurological deficit. SLRS was 70 degrees in the right with pain.

The old wound was refreshed and the right upper edge of the bony approach to L5-S1 was also refreshed until the extruded material was identified. It was removed in one piece.  The shallow collapsed disc space of L5-S1 was cleaned from remnants of disc material. Vo attempt was done to remove the scar and only the root edge was seen.

Routine closure of the wound and smooth postoperative recovery.

Recurrence was expected in the first operation to be higher than 7%, because the disc space was not shallow at that time. It was good idea to use the PEEK Satellite sphere at that time to reduce the recurrence.

At this stage using the PEEK Satellite is meaningless, since the disc space became shallow and the expected recurrence rate now is below 2%.
