RECURRENT PLD L4-5 MORE TO LEFT.
Anamnesis:
The patient was operated
by me 23-September-2008
for PLD L4-5 for left sciatica. The patient
improved then she came 01.January-2009
complaining of left sciatica for the last 10
days.
MRI lumbar spine performed
30-December-2008 showed huge central recurrence at L4-5
more to the left
side.
The patient had exaggerated
scoliotic stance with SLRS was 90 degrees both
sides and without neurologic deficit.
Scarolysis of the left L5
root was performed and drilling was achieved, so
the the extruded disc was identified. The
extruded disc was removed lateral to the axilla
and meticulous cleaning of L4-5 disc space was
performed from the left side. Routine closure of the wound.
Smooth postoperative
recovery.
Comments
The patient had in the first
surgery wide defect in the annulus fibrosis and
the disc space was not shallow for what the
estimated recurrence rate above 7%.
Now the disc space is
shallower than before, for what the estimated
recurrence rate is less than 7%.
