18-MARCH-2008  MUNA MUHAMED AL-SALEH 34 YEARS  EXTRUDED DISC L4-5 WITH SEVERE LEFT L5 ROOT COMPRESSION.

Anamnesis:

The patient came to the clinic 16-March-2008 complaining of LBP for 5 years with intermittent course. The last 2 weeks, exacerbation of LBP with left sciatica  with left L5 territory.

MRI lumbar spine done 11-March-2008 showed extruded disc L4-5 with compression of the left L5 root. There is sacralization of the L5 body.

On examination: the patient has exaggerated scoliotic stance with SLRS right 40 degrees with shooting pain to the left leg and 20 degrees in the left. There is weak dorsi and planterflexion left foot 4/5.

Left L4-5 hemiflavotomy with left L5 root foraminotomy was performed. The epidural fat was missing at the compressed areas. The root was shifted medially and the extruded disc material was removed in several pieces. Cleaning of the disc space was performed from the left side.

Routine closure of the wound with smooth postoperative recovery.

The power of the left foot returned to normal.

The height of the disc space is relatively medium to small, which make the expected recurrence rate less than 7%.
