EXTRUDED DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.

Anamnesis
The
patient came
to the clinic 16-August-2010 complaining of
LBP for three months with right sciatica for for
one month down to the right S1 territory.
MRI lumbar spine
performed 16-August-2010 showing extruded disc
L5-S1 with right downward migration.
On examination: the
patient is limping with exaggerated scoliotic
stance. SLRS was 70 degrees with pain in the
right. There was weak dorsiflexion -4/5 and mild
weak planterflexion right foot.
Right L5-S1 hemiflavotomy with right S1
foraminotomy was performed. The extruded disc
was removed lateral to the axilla. The severely
compressed root regained lax position. Right sided
cleaning of L5-S1 space was achieved. The
epidural fat was transferred from the midline to
the direction of the right S1 root to minimize
the postoperative adhesions around the root.
Smooth postoperative recovery and improvement of
the power of the right foot.
Comments
The estimated postoperative
recurrence rate in this case is around 7%,
because the disc space is not shallow.
The extruded disc was hard in
consistency and dark in the MRI, which means
that, it will never shrink over time.
