19-NOVEMBER-2008 HAMED MUHAMED AL-BARBARY 32 YEARS EXTRUDED DISC L5-S1 WITH
RIGHT UPWARD MIGRATION.
Anamnesis:
The patient came directly to
the operating room with clinical picture of
agonizing right sciatica for 2 months with weak
dorsi and planterflexion right foot and numbness
of the dorsum of the same foot.
MRI lumbar spine performed
11-November-2008 showing extruded disc L5-S1
with right upward migration.
Right L5-S1 hemiflavotomy
with right S1 foraminotomy was performed. The
extruded disc was attacked lateral to the
axilla. There were very huge epidural veins,
which were preserved. Right sided cleaning of
the disc space of L5-S1 was achieved. Routine
closure of the wound.
Smooth recovery with prompt
improvement of the power of right foot.
Comments
The estimated recurrence rate
in this case is around 7%, because the disc
space is not shallow and the defect in the
annulus fibrosis not a small one.
