The patient with degenerative spine came complaining of severe left sciatica with drop left foot. MRI lumbar spine showed extruded disc L4-5 with left down migration. The patient was operated. Left L4-5 hemiflavotomy was done with foraminotomy left L5 root. The sequestrated disc was removed in parts, some from above the axilla and parts from below. No attempt was made to perform meticulous cleaning of the intradiscal space. The sciatica disappeared and the power of the left foot improved dramatically,
