Surgical treatment in
paraplegia survey:
Cross-anastamosis in paraplegia below D9 started to give
results. The last documented case operated 1 year ago in a patient from Israel came
to the clinic 3 weeks ago. ECS and EMG performed showed that there is
starting innervation of Th 11 and 12. The patient's lower limbs muscles
became bulky and he could contract the lower abdominal muscles and some
movements in the pelvic girdle. Crude sensation descended down to the

Tuberculosis of the
spine
In the last 2 years the incidence of
tuberculosis of the spinal column is becoming more frequent and having
different clinico-morphologic picture. This phenomenon is alarming sign
as the residual of the use of dirty bombs and several radioactive

AMAL MAHMOOD ABDEL-QADER 31 YEARS EXTRUDED DISC C6-7
LEFT MIDIO-LATERAL.


Anamnesis
 |
The patient came to the
clinic 05-March-2007 complaining of neck pain
for 18 months with right radicular pain for 9
months and numbness both hands for 3 weeks.

MRI of the cervical spine
performed 03-March-2007 showing a huge extruded
disc C6-7 with left midio-lateral localization. |
 |
On examination; the patient
had limitation of neck movement with
severe weak flexion and extension of both
hands and both triceps muscles. She had positive
Hoffmann sign both sides, for what brain CT-scan
was performed and it was normal. The patient
was advised to undergo surgery. |
 |
Discectomy of C6-7 was
performed through anterior approach and after
cleaning of the disc space at C6-7, the defect
of the posterior longitudinal ligament was
widened to make the extrusion coming out and
removed. Several fragments were removed. |
 |
The dura was glistening
relaxed and check of the foramena was negative
for remnants. |
 |
Routine closure of the wound
and smooth postoperative recovery with
disappearance of the radicular syndrome. |
Comments:
 |
The bony elements practically
were untouched and the single level all make
unnecessary to put artificial devices, such as
artificial disc or fusion devices of several
types. Here the presence of Leuscko joints in
the lateral aspect of the disc space, provide
the stability, taken together with other
supporting elements. |

The good news in cervical
disc surgery, that, there is no recurrence in
comparison to the lumbar disc which still having
recurrence rate around 7-10%. |
