Surgical treatment in
paraplegia survey:
Cross-anastamosis in paraplegia below D9 started to give
results. The last documented case operated 1 year ago in a patient from Israel came
to the clinic 3 weeks ago. ECS and EMG performed showed that there is
starting innervation of Th 11 and 12. The patient's lower limbs muscles
became bulky and he could contract the lower abdominal muscles and some
movements in the pelvic girdle. Crude sensation descended down to the

Tuberculosis of the
spine
In the last 2 years the incidence of
tuberculosis of the spinal column is becoming more frequent and having
different clinico-morphologic picture. This phenomenon is alarming sign
as the residual of the use of dirty bombs and several radioactive

AHMAD MUHAMED AL-ZEER 55 YEARS POSTTRAUMATIC
SLIPPED MOST RIGHT UPPER SCREW OF THE HYBRID CERVICAL
PLATE.
Anamnesis
 |
The patient was operated for
posttraumatic multilevel segmental instability
of the cervical spine more than one year ago and
check X-rays were performed several times for
the construct were acceptable. |

The patient suffered another
RTA 25-August-2007 after what he started to
complain of difficult swallowing. |
|
|
Check X-ray after trauma
showed slipping of the most right upper screw in
the construct. |
|
|
So as to avoid possible
repetitive trauma to the esophagus, it was
planned to remove the screw and put replace it
with more wider diameter one with the same
length. |
|
|
Using the same previous
incision, the tip of the screw was identified,
guided by image-intensifier, trying during that
not to traumatize the esophagus or other
important structures in the scarous field. The 4
mm diameter 16 mm length screw was replaced with
16 mm length 4.5 mm diameter and it was not
loose. |

|
Smooth postoperative
recovery. |
Comments:

The patient was exposed
to sheering force during accident and momentum
was accumulated at the right upper part of the
construct, which caused the slipping of the
screw by counter-shock mechanism. The Reflex
Hybrid system is designed to prevent such
accidents, but any construct, whatever its
advantages, cannot stand extreme circumstances. |
|
|
The purpose of the surgery is
to prevent repetitive trauma during swallowing
so as not to have esophageal fistula or tear,
which could lead to disastrous sequelae. |
