The patient came to the clinic
20-July-2006 complaining of LBP with right sciatica for 1 month with
scoliotic stance. The patient is limping for 12 days with SLRS 60
degrees in the right , hypalgesia right L5 root and almost drop
right foot. MRI performed 17-July-2006 showing a huge extrusion of
L4-5 disc with back and downward migration. Right L4-5
hemiflavotomy was performed and foraminotomy of right L5 root was
achieved. All the disc material was removed from under the axilla,
after what the laterally pushed nerve regained relaxed appearance.
The annulus fibrosis was inspected and there was no tear or defect
in the annulus, for what it was decided not to violate the
intradiscal space.
Comments: 1. The slippage
mostly took place from the inferior border of the annulus in the
right side. Time will answer if this strategy is logic to minimize
the recurrence rate. |
