The patient came to the clinic 20-July-2010 complaining of neck and left shoulder pain with numbness all fingers left hand for 45 days.

MRI cervical spine performed 12-July-2010 showing PCD C5-6 central and C6-7 with left foraminal occlusion with bulging C4-5 with compression of the spinal cord at C6-7 level.

On examination: the patient has weak grip left hand 4/5 extension left hand 3/5, the triceps left arm 2/5 and the right arm 3/5.There is no frozen shoulder and the patient is a known diabetic for 6 years.

Discectomy of C5-6 and C6-7 with removal of the compressing fragments at the left foramen of C6-7. Insertion of Fidji cages 17 mm width 6.3 height and 12 mm depth was inserted to the C5-6 level with novabone. Another cage 17 mm width, 6.1 mm height and 12 mm depth was inserted with novabone to the C6-7 level. Using Trinica Zimmer miniplate 2 level 44 mm length was used to fuse C5-6-7. Image-intensifier was used at all stages of the operation.

Routine closure of the wound and smooth postoperative recovery.

The power of both upper limbs dramatically improved immediately after surgery.

There are a lot cervical miniplates in the market. Some has some advantages and disadvantages. It seems that this brand is ranking in the high standard of its specifications. Time will tell more in the future.

It became a usual practice to put the cages with the miniplates at the discectomy site to avoid future stress in the construct and keep the anatomical alignments, to avoid sandwich compression at the foramen level . Fidji brand is designed to conform with the space of the disc cavity, thus the upper and lower endplates of the removed disc will fit exactly with construct.
