The patient came to the clinic
20-July-2006 complaining of LBP with left sciatica for 1 year with
mild scoliotic stance. Exacerbation of sciatica the last 2 weeks.
MRI lumbar spine done 15-July-2006 showing PLD L4-5 with extrusion
and left downward migration. On examination: SLRS was 45 degrees
in the right and 40 degrees in the left. He had weak dorsiflexion
all toes left foot.
Left L4-5 hemiflavotomy with left L5 foraminotomy was performed.
The extrusion was removed lateral to the axilla and meticulous
cleaning of the disc space was performed from the left side, because
the defect in the annulus fibrosis was wide. The downward migrated
fragment was removed in one piece after inspection of the root.
Prompt postoperative recovery.
Comments: 1. Since the
defect in the annulus fibrosis was wide, it was necessary to perform
meticulous cleaning of the disc space, to decrease the recurrence. |
