The patient came to the clinic 24-August-2008 complaining of neck and right shoulder pain for three years with exacerbation last month with numb index right hand.

MRI performed 26-July-2008 showed PCD C6-7 with right foraminal occlusion.

On examination: the patient has weak extension of the right hand  the weak right triceps muscle 3/5. The patient has pain when looking down and bending the head to the right with agonizing pain with sleep disturbances due to pain.

Anterior approach with discectomy of C6-7 was done under image-intensifier. The extruded material was removed in several pieces.

Routine closure of the wound and smooth postoperative recovery with normalization of the power of the right upper limb.

The patient was treated by traction 3 weeks ago and he noticed that traction increased the pain. When the extrusion is big as in such case it is preferable to avoid traction, so as to prevent catastrophic complications.

Surgery as in this case must be performed as soon as possible.

Simple discectomy is the preferred choice of treatment, since there is no signs of instability.
