26-JANUARY-2008  AYMAN MUHAMED NADDAF  32 YEARS  HUGE RECURRENT PLD L4-5 LEFT SIDE.

Anamnesis:

The patient was operated by me for PLD L4-5 left side 23-July-2007.

The patient came to the clinic 07-August-2007 with still scoliotic stance with normalization of the power ob both feet. The patient over the time showed some improvement, but then came 05-December-2007 with the same sciatica and normal power of the feet.

The patient was sent for check MRI, which was performed 19-January-2008, showing huge recurrence of the disc at the same site. The patient was advised to undergo surgery for the recurrence.

Neurolysis of the left L5 root and exposure of the recurrence lateral to the axilla. The extruded disc was removed in one piece and further cleaning of the disc space of L4-5 was performed from the left.

Routine closure of the wound and smooth postoperative recovery.

Comments

As previously mentioned, the expected recurrence in this case was medium.  The recurrence was mostly several days after the surgery.

bullet

The neurological status of the patient was normal. The presence of pain must hold suspicion about the recurrence, since the roots are not severely compressed after foraminotomy. This is the usual finding of all recurrences.

bullet

The disc space now is shallow, which means that the expected re-recurrence is below average.
