PLD L4-5 WITH LEFT SIDED EXTRUSION.
Anamnesis:
The patient came to the
clinic 20-August-2008 complaining of LBP for 2
years and left sciatica for 4 months.
On examination: the patient
dragging the left leg with exaggerated scoliotic
stance and limping. SLRS was 50 degrees in the
left with pain. There is weak dorsiflexion left
foot -4/5.
MRI performed 21-August-2008
showed PLD L4-5 with left sided extrusion with
bulge L5-S1.
Left L4-5 hemiflavotomy with
foraminotomy of left L5 root was performed. The
extruded disc was removed and meticulous
cleaning of the disc space was performed from
the left.
Routine closure of the wound
and smooth postoperative recovery with
normalization of the power of the left foot.
Comments
The expected recurrence rate
in this case is around the average, because the
disc space still not shallow.
Local transfer of the
epidural fat is the best choice to cover the
exposed root, to minimize the postoperative
fibrosis.
