The patient came to the clinic 24-August-2009 complaining of inability to set and walk after falling down from 150 cm height 45 days ago. The patient was admitted to the University hospital and sent home undiagnosed.

MRI lumbar spine performed 22-August-2009 showing burst fracture of L2 and wedging of L1.

The patient could walk with the help of 2 persons and he could set with his upper arms elevating the body. He had weak both lower limbs 4/5 without sensory deficit.

Skeletonization of D11-12-L1-2-3 and L4  was performed, preserving during that the spinous processii and interspinous ligaments. Using Medtronic sublaminar hooks and rods distraction reduction was achieved and fusion was performed between D11-12 and L3 and L4.

Medtronic Bicalphos  granules 20 cc 1.6-3.2 mm was applied into both sides after decortication of the bones mainly to the D12 L2 levels.

Routine closure of the wound and smooth postoperative recovery and the patient sent to the ward.

The patient has burst fracture of L2 with the patient unable to walk or set down with agonizing pain. Fixation was mandatory 2 levels above and 2 levels below the affected lesion which was the fractured L1 and L2.

This granules were inserted in case that the construct suffer fatigue and ossification will help in stabilizing the fractured bones.
