The patient started to complain of neck and both shoulder pain for more than 6 years. He also started to complain of LBP for one year . MRI performed earlier and recently showed that he has CCS with OPLL and PCD C3-4 and C6-7 with compression of the spinal cord at
these levels, causing weak left hand grip, extension and left triceps muscle. MRI of the lumbar spine was not remarkable. The patient was operated and discectomy of C3-4 done with removal of the OPLL with subsequent fusion, using one level miniplate. The cages performed earlier were abandoned due to their failed to test construction and their frequent slippage and difficulty of their insertion through small incisions. The PCD C6-7 was cleaned and the extrusion was removed . Postoperative course was uneventful, and the patient regained full power.
