ABU-DIYE 24 YEARS EXTRUDED DISC L4-5 WITH SEGMENTAL STENOSIS.
Anamnesis
The patient came to the clinic 26-Deceember-2004 complaining of LBP
for 14 months with right sciatica for 10 months. MRI lumbar spine
performed 14-December-2004 showing extruded disc disc L4-5 with
downward migration.
On examination: The patient is limping with exaggerated scoliotic
stance. SLRS was 75 degrees with more pain in the left. There is
hypalgesia left L5,S1 roots territories. Weak dorsiflexion left
foot.
Decompressive laminectomy of
L4-5 with bilateral foraminotomy both L5 roots.
Bilateral cleaning of the disc space and removal
of the extrusion. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
He was sent to the ward.
Follow Up
The patient came 18-January-2005 with clean wound and SLRS 60
degrees both sides and no motor, nor sensory deficit and came several times the next 12 years and he was
neurologically free.
He came last time 10-April-2017 with sudden onset 10 days ago of
left sciatica and weak dorsiflexion left foot 4/5 and sent for MRI
lumbar spine, which done 17-April-2017 showing small fragment the
the left foramen of L4-5 and he was advised for conservative
treatment.
Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space height is not
shallow.
