06-JULY-2003  MARIAM AHMAD AQEL 60 YEARS LUMBAR CANAL STENOSIS L2-3, 3-4, 4-5 WITH DROP FEET.

 

Anamnesis

The patient came to the clinic 06-April-2003 complaining of LBP with left sciatica for 5 years with exacerbation last month with bilateral sciatica. Hypertensive for for 2 years in treatment. MRI lumbar spine performed 18-August-2001 showing LCS L2-3, 3-4 and 4-5.

On examination, the patient limping with waddling gait, bended anterior with exaggerated scoliotic stance. There is weak quadriceps muscles 4/5 abduction and adduction of the knees 3/5. flail left foot for 3 years and the right after that.

The patient was sent for investigation and MRI lumbar spine repeated 26-May-2003 showing severe stenosis L2-3, 3-4 and L4-5.

Decompressive laminectomy L3,4 and upper half of L5 and lower half of L2. Foraminotomy all L3,4,5 roots both sides. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Routine closure of the wound.

Smooth postoperative recovery. The patient showed some improvement of the power of her proximal muscles both legs. She was sent to the ward.

FOLLOW UP

The patient came to the clinic 20-July-2003 with clean wound. The power of the proximal muscles became full 5/5, but still drop both feet.

The patient then came 06-May-2007 with further improvement: the dorsiflexion right foot 3/5 and planterflexion right foot 4/5, planterflexion left foot 3/5, but drop left foot.

The patient then came 17-November-2008 telling that she was improving, but the last 3 months got right hip pain and walking with walker. MRI lumbar spine performed 26-October-2008 showing lumbar canal stenosis L1-2. with extruded disc at this level. She was advised to perform surgery.

 

Comments

The patient came too late with drop left foot for 3 years. In this case the recovery of this drop left foot mostly will be zero.
