Anamnesis
The patient came to the clinic 26-January-2004
complaining of pain of the thumb left hand for 5
months. The patient underwent hemithyroidectomy
1990.She is diabetic for 2 years. There is LBP
with left sciatica.
On examination: The patient is in pain, limping with exaggerated scoliotic stance.
There is weak dorsiflexion left foot -4/5,
planterflexion same foot 4/5. SLRS was 80 degrees with pain left
side. There is hypalgesia left L5, S1
territories.
The patient was sent for investigations and MRI
lumbar spine performed 09-January-2005, showing
extruded disc L4-5 with left upward migration.
Extended left L5 foraminotomy
with removal of the left far upward migrating extrusion and left sided
intradiscal cleaning of L4-5. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
She was sent to the ward.
Follow Up
The patient came 26-January-2005 with clean
wound and SLRS was 80 degrees both sides and
neurologically free. The she came 05-July-2005 complaining of LBP
with mild right sciatica. Neurologically she was
free and MRI lumbar performed 11-July-2005 ruled
out recurrence.
Comments
The recurrence rate is around 7%, because the
disc space still not completely collapsed.
