21-JANUARY-2005 NAJWA MUHAMED ABDO 61 YEARS SEVERE CERVICAL STENOSIS C4-5-6-7
WITH QUADRIPARESIS.
Anamnesis
The patient came to the clinic 17-January-2005
complaining of neck pain for 2 moths with
exacerbation last month with numbness of the
left hand with heaviness both legs. MRI cervical
spine performed 23-December-2004 showing severe
cervical stenosis C4-5-6-7 with malacia of the
spinal cord at C4-5 level.
On examination: The patient is limping with
ataxic gait. There is weak flexion, extension
both hand and both triceps muscles 3/5. There is
hypalgesia lateral aspect left arm. Hoffmann
sign positive more brisk in the left. All the
muscles of both legs are weak 4/5.
Through anterior approach
discectomy of all levels with fusion of C4-5-6-7 was
achieved. Routine
closure of the wound.
Smooth postoperative recovery.
She was sent to the ward.
Follow Up
The patient came to the clinic 03-February-2005
with dramatic improvement of the power of four
limbs with mild regression of the hypalgesia.
The last visit 24-April-2006 came with new
complain and MRI dorsal spine involved with
multiple myeloma and sent to hematologist.
Comments
Here the anterior approach was in favor,
because the extruded disc C4-5 must be dealt from anterior.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
