The patient came to the clinic 08-January-2019 complaining of neck pain with left upper limb pain for 6 months. The last week got right upper limb pain and numbness of the thumb and index finger right hand. MRI cervical spine performed 03-January-2019 bad quality showing extruded disc C5-6 bilateral and C6-7 more to the right with right foraminal occlusion. The patient is a known diabetic for 12 years.

The patient had neck pain when looking up and to the right. There was weak grip left hand 4/5, extension of the right hand -4/5, left hand extension 4/5 and right triceps 3/5, left triceps 4/5 with hypalgesia thumb and index right hand. She was sent for investigation.

MRI cervical performed 08-January-2019 showing huge extruded disc C5-6 more to right with right foraminal occlusion and C6-7 broad based with spinal cord compression. ESR was 25 mm/h and CRP was 7 mg/L. The patient was advised to start Zinnat 500 mg twice daily to minimize the incidence of postoperative infection.

Discectomy of C5-6 and C6-7 with removal of the extrusion at the both corners of the field until the dura was seen all over. Insertion of OSIMPLANT EOS cervical bladed cages 13x15 H6 with IN'OSS bone graft to both levels. Routine water-tight closure of the wound.

Smooth postoperative recovery. The power of both upper limbs normalized. She was sent to the ward.

The recurrence in cervical discectomy is zero. In rare cases when the patient progress OPLL, the mentioned pathology could progress, masking the picture of recurrence after long period of time.
