03-JANUARY-2012 AFAF IBRAHEEM ABBAS 51 YEARS EXTRUDED
DISC L5-S1 WITH WIDE-BASED EXTENSION.
Anamnesis
The
patient came to the clinic 06-December-2011
complaining of LBP for 4 years with right
sciatica for 2 months down to gastrocnemius.
MRI of the
lumbar spine performed 30-October-2011 showing
bulge L4-5 and wide-based extrusion L5-S1
central more tot eh right and left downward
migration.
On
examination, the patient has
exaggerated scoliotic stance. There is weak
dorsiflexion right foot 3/5 and planterflexion of
the same foot 4/5. SLRS was 80 degrees in the right with
tightness and 80 degrees in the left with pain.
The right AJ is decreased.
Bilateral L5-S1 flavotomy
with foraminotomy of the right S1 root far down
to regain good exposure of the downward
migrating piece. The extruded disc was removed
subaxillary from the right. Further cleaning of
L5-S1 disc space was performed. Inspection of
the left S1 root, ruled out the necessity to
violate the disc space from the left.
Routine
closure of the wounds. Smooth postoperative
recovery with improvement of the power of the
right foot.

Comments
The patient still having recurrence rate below
7% because the disc space is slightly shallow.
