The patient came to the clinic 12-February-2002 complaining of ataxia for 3 months after CVA. MRI of the brain done27-October-2001 was normal. The patient is unable to walk for 3 months. He has bronchial asthma and Parkinson's disease under treatment.

On examination: The patient is in wheelchair, with tetraparesis and tetra-analgesia with Hoffman sign both upper limbs and Babinski positive right side.

The patient sent for investigations and MRI cervical spine performed 14-February-2002 showing huge extruded disc C4-5 with severe spinal cord compression.

Through anterior approach, discectomy of C4-5 with removal of the extrusion until the dura was seen transparent at the posterior area of the cleaned disc cavity. Routine closure of the wound.

Smooth postoperative recovery. The power of four limbs dramatically improved and he could walk the same day after surgery.

The patient the came to the clinic 26-February-2002  with clean wound walking with no sensory nor motor deficit.

The patient then came 12-August-2002 complaining of numbness four limbs  with pathologic reflexes still the same, but no sensory, nor motor deficit. He was given medications for Parkinson's disease and other medications to decrease the numbness.

The patient had wrong diagnosis of CVA. He had initially extruded disc C4-5, which happened during the ceremony of the marriage of his son.
