21-JUNE-2014 AMAAL YASEEN BATARNI 57 YEARS
SPONDYLOLISTHESIS L4-5 WITH SEVERE SEGMENTAL STENOSIS.
Anamnesis
The patient came to the clinic 17-June-2014
complaining of LBP for 2 years with right
sciatica for 1 day with numbness planter side of
the right foot.
On examination; The patient is
limping with exaggerated scoliotic stance. SLRS
60 degrees with pain in the right. There is weak
dorsiflexion right foot -3/5.
The patient was sent for investigations and MRI
lumbar spine done the same day showing
spondylolisthesis L4-5 with severe segmental
stenosis with bulge disci L3-4 and L5-S1.
Dynamic studies also confirmed the
spondylolisthesis of L4-5.
Laminectomy L4 and upper third of L5 with
foraminotomy right L5 root. Discectomy of L4-5
with cleaning from the right. Insertion of TLIF
cage Vitatech 30x9 with bone graft to both
sides. 4 monoaxial 6.5x45 mm transpedicular
screws were inserted to the L4 and L5 bodies. 2
bended rods and cross connector were used to
fuse L4 and L5. The harvested bone was used
lateral to the rods. Routine closure of the
wound.
Smooth postoperative recovery. The power of the
right foot became normal and the agonizing right
sciatica disappeared.
Comments
The patient has minimal spondylolisthesis
because there is spinal stenosis at this level. All these
factors taken together, necessitate the surgical correction.
