The patient came from Bahrain to the clinic 08-January-2006 complaining of severe severe neck pain for one month with left shoulder pain and numbness of the left hand. On examination, she had severe weak grip and extension left hand with weak left triceps muscle with limitation of neck movement to most directions.

The patient was sent to MRI cervical spine the same day and a huge extrusion C6-7 disc with migration to the left foramen was seen.

The patient was operated and through small incision, anterior approach was applied. After discectomy C6-7, the PLL was removed from the medial part and the extrusion was removed in several big pieces. The PLL was removed far lateral to the left and check of the free foramen was reassured.

Prompt postoperative recovery with normalization of the power of the left upper limb.
