30 YEARS EXTRUDED DISC L4-5 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came
to the clinic 30-January-2002 complaining of LBP
with left sciatica for 8 months.
MRI of the lumbar
spine done 17-January-2002 showing extruded disc L4-5 left side with
downward migration.
On examination: The patient
is limping with exaggerated scoliotic stance
with SLRS was 40 degrees with pain left side and
weak dorsi and planterflexion left foot -4/5.
Foraminotomy left L5 roots. The disc of L4-5 was removed from
left side of the axilla and meticulous cleaning
of L4-5 disc space was achieved. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
He was sent to the ward.
Follow Up
The patient the
came to the clinic 17-February-2002 with
clean wound with improvement of the power of the
left foot.
The patient then came 01-April-2003 complaining
of left sciatica for 1 week. SLRS was 45 degrees
with pain, but no motor or sensory deficit. The
patient given medications and sent for
investigations, but he disappeared.
