23-JULY-2007 AYMAN YOUSEF NADDAF 32 YEARS HUGE PLD L4-5 WITH LEFT DOWNWARD
MIGRATION.
Anamnesis
The patient came to the clinic 21-July-2007
complaining of LBP for 3 years without sciatica.
Tow months ago, got left sciatica with numbness
of the left L5 and S1 territory.
MRI performed 08-July-2007 in Qatar showed a
huge extruded disc L4-5 with downward migration
left side.
On examination: the patient is limping and has
scoliotic stance with SLRS right side 45 degrees
with pain and diminished AJ left side. There is
weak dorsi 3/5 and planterflexion 4/5 left foot.
Flavotomy L4-5 with left L5 root foraminotomy
was done. Most of the extrusion was removed from
under the axilla. Meticulous cleaning of the
disc space was performed from the left side.
Inspection of the right side was negative.
Routine closure of the wound.
Smooth postoperative recovery.
Comments
The disc space was medium in height, which
predict a medium rate of recurrence.
As usual, the epidural fat was transferred to
the root from the nearby structures, to prevent scar formation
around the root area.
