08-MAY-2004 AZMI HASAN AL-ABBADI 36 YEARS HUGE
EXTRUDED DISC L5-S1 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic
04-May-2004 complaining of LBP with left
sciatica for 8 months with right sciatica for
2 months down to the right S1 territory. MRI
lumbar spine done 06-March-2004 showing huge
extruded disc L5-S1 with right downward
migration. Hypertension for 2 years and
cath done 2 years was free.
On examination: The patient is limping with
exaggerated scoliotic stance. SLRS was 75 degrees
in the right with pain. There is
weak dorsi and planterflexion right foot 3/5 with hypalgesia
right S1 territory.
Foraminotomy
right S1 root with
removal of the extrusion lateral to the axilla. After
that, the root became lax and right sided
intradiscal cleaning L5-S1 disc space was done.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. His own epidural
fat was preserved to
minimize postoperative scarring. Routine closure
of the wound. Smooth postoperative recovery. The
power of the right foot became normal. He was
sent to the ward.
FOLLOW UP
The patient came to the clinic 17-May-2004
with clean wound and SLRS 90 degrees both sides
without pain and no motor, nor sensory deficit.
The patient then came 21-July-2005 complaining
of numbness right S1 territory with right hip
pain, but neurologically he was free. MRI
cervical spine performed the same day showing
small bulge C5-6.
Comments
The estimated postoperative recurrence is still
ranking around 7%, because the disc space is
still relatively not shallow.
