40 YEARS HUGE EXTRUDED DISC L5-S1 RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic
09-September-2002
complaining of LBP with right sciatica for 2 months.
On examination: The patient in agonizing pain,
using crutches and
limping with exaggerated scoliotic stance.
SLRS
40 degrees
with pain in the right. There is severe weak dorsi planterflexion
right foot 3/5.
He was sent for investigations and MRI lumbar
spine performed 11-Sptember-2002 showing huge
extruded disc L5-S1 with right downward
migration.
Right S1 foraminotomy with removal of the
extrusion lateral to the axilla from the
right. Right sided intradiscal cleaning L5-S1.
The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 28-September-2002
with clean wound and neurologically free with
SLRS 70 degrees without pain both sides.
The patient then came several times asking for
reports and the came 08-December-2018 with new
MRI performed the same day showing no recurrence
Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space is still not
shallow.
