17-MARCH-2014 FATHI YOUNIS AL-SAYD 54 YEARS
EXTRUDED DISC L4-5 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 15-March-2014
complaining of LBP for 2 months with
exacerbation of LBP and right agonizing sciatica
for 10 days. MRI lumbar spine done 02-March-2014
showing extruded disc L4-5 with central more to
the right and downward migration. The patient
diagnosed as having diabetes mellitus for 3
months. Arterial hypertension for 15 years.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 70
degrees left side and 20 degrees with pain in
the right. There is drop right foot 3/5 and
planterflexion same foot 3/5 with hypalgesia
right L5 and S1 territories.
Using C-arm, the L4-5 level was identified.
Right L5 foraminotomy was done and removal of
the extrusion was performed lateral to the
axilla. Right sided intradiscal cleaning L4-5
disc space. Routine closure of the wound.
Smooth postoperative
recovery. The power of the right foot became
better.
Comments
The patient still has an estimated
postoperative recurrence rate around 7%, because
the disc space height still not shallow.
