EXTRUDED DISC L4-5 WITH RIGHT UPWARD MIGRATION.
Anamnesis
The patient was operated by me
13-May-2006 for
huge extruded disc C5-6. The patient then came to the clinic 03-November-2015
complaining of LBP for 2 weeks with right sciatica
down to the toes.
On examination: the patient is limping with
exaggerated scoliotic stance. SLRS was 70 degrees with pain
right side. There is weak dorsiflexion right
foot 3/5.
The patient was sent for investigations and MRI
done 03-November-2015 showing bulge diski L2-3,
3-4, L5-S1 with extruded disk L4-5 with right
upward migration. Dynamic studies were normal.
Right L5 foraminotomy was
performed and the extruded upward migrating
extrusion was removed and right sided
intradiscal cleaning was achieved. Using MultiGen, a pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 mesec
duration the right L5 root was achieved. Routine closure of the
wound.
Smooth postoperative recovery.
The power of right foot became normal. The patient was
sent to the ward.

Comments
The patient still has an estimated
postoperative recurrence around 7%.
This is the 20th case using the PRF mode
with MultiGen. This procedure regained routine acceptance
and it will not be mentioned in the future, since it will be
a usual part of the disc surgery.
