11-MARCH-2015 FUAD FAROOQ ASLAN 53 YEARS
EXTRUDED DISC L4-5 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 08-March-2015
complaining of LBP with agonizing left sciatica
the last 4 weeks down to calf muscle. The
patient using crutches for 2 weeks.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 60
degrees in the right and 5 degrees in the left
with pain. There is
weak dorsiflexion left foot -3/5 and
planterflexion same foot
-4/5. The patient is a known hypertensive for 10
years and cardio cath done 2007 was normal.
The patient sent for investigations: MRI of the
lumbar spine done 01-February-2015 showing huge
extruded disc L4-5 with left upward migration
with segmental stenosis. The patient was sent
for cardiologic consultation.
The patient is fatty, for what the L4-5 level
was identified several times using the C-arm
Foraminotomy left L5 root. The downward extruded
disc was removed lateral to the axilla. Left
sided intradiscal cleaning L4-5 space.
Inspection of the the subaxillary area.
Routine closure of the wound.
Smooth postoperative recovery. The power of left
foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
