GHADA ANWAR MUHAMED AL-SULTAN 74 YEARS BILATERAL ACUTE
AND SUBACUTE SUBDURAL HEMATOMA BOTH CEREBRAL HEMISPHERES.
Anamnesis
The patient was admitted to the ICU Shmaisani
hospital with severe headache, drowsiness,
frequent falls, disorientation and abdominal
pain for 1 month. CT-scan done today showing
bilateral subdural hematoma.
On examination, The patient is talking, moving
all limbs. No signs of conning, disoriented but
with somnolence.
MRI of the brain was performed showing bilateral
subdural hematomas of different ages extending
from the frontal pole down to the right
occipital convexity and the left parietal
region.
Lab investigations were uneventful. CRP was 17
mg/L, ESR 55 mm/h.
In semi setting position with
the head neutral, bur holes were inserted behind
the frontal hear line, 8 cm away from midline
and other 2 bur holes in the right posterior
occipital and left posterior parietal
projections. Insertion of external drains to
both sides and evacuation of the hematomas. The
right was bloody and the left was xanthochromic.
Washing of the hematomas until the external end
showed clear solution. Irrigation of both sides
showed communication of both hematomas. 2
sets of Medtronic external drains were
used one for each side, Routine closure of the
wounds. The patient showed normal recovery. She was sent to the
ICU.
FOLLOW UP
Too early now.
Comments
The patient have huge hematomas, and the
sooner the evacuation of the hematomas, the better the
outcome.
