HASHEM MUHAMED AL-SELAWY HUGE RECURRENT EXTRUSION L5-S1 WITH RIGHT UP AND DOWNWARD
MIGRATION.
Anamnesis
The patient came to the clinic 25-May-2015
complaining of LBP for 10 months with
exacerbation of right sciatica 2 months after
performed discectomy L5-S1 one year ago in
Turkey Adana. MRI lumbar spine performed
07-April-2014 before surgery, showing huge
extruded disc L5-S1 with right downward
migration.
On examination; the patient is limping with
exaggerated scoliotic stance. Neck and upper
limbs were neurologically free. SLRS was 20
degrees with pain right and 60 degrees with pain in the
left shooting to the right. There is weak dorsiflexion
both feet -4/5. Weak planterflexion right foot
-4/5. The right AJ is absent.
The patient was sent for investigations and MRI
performed 25-May-2015 showing huge recurrence at
the same previous location.
Using the old incision, dissection was carried
out and a small tear at the scar was seen at the
right side with nylon. The CSF came out and a
root was seen in the field. The C-arm showed the
level 2 cm below the supposed to be L5-S1. The
dissection was carried up and drilling was done
to achieve approach to the L5-S1 disc space. The
extruded disc was removed and the up and
downward migrating pieces were removed and
further cleaning of L5-S1 disc space was
achieved. The scareous dural tear was closed
using 4 zero nylon. Routine
closure of the wound.
Smooth postoperative recovery. The power of both
feet became normal.
Comments
The patient still having an estimated
postoperative recurrence below 7%.
