LCS L2-3, 3-4, 4-5 AND L5-S1.
Anamnesis
The patient came to the clinic
03-March-2002
complaining of LBP for 5 years with left
sciatica for 3 months. She is using crutches for
3 months with pain both knees with inability to
turn her self during sleep due to pain. The
patient is hypertensive for 5 years. MRI lumbar
spine performed 20-June-2001 showing severe LCS
L2-3, 3-4 and L4-5.
On examination: The patient is in agonizing
pain, limping, walking bended, with exaggerated scoliotic stance.
There is weak dorsi and planterflexion both feet -4/5
more the right. SLRS was 90 degrees both sides.
The patient was sent for investigations and
MRI lumbar spine showed severe stenosis
L2-3,3-4,4-5 and L5-S1.
Decompressive laminectomy L2,3,4,5 with lower
half of L1 with foraminotomy L2,3,4,5 and S1 roots
both sides. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
She was sent to the ward.
Follow Up
The patient came to the clinic
17-February-2002 without crutches with clean
wound with full power both feet.
The patient then
came 10-March-2004 complaining of neck pain with
left upper limb pain with weak grip left hand
and left triceps muscle. She was sent for
investigations and given medications, but she
did not show up.
Comments
The sooner
the surgical intervention, the better the outcome, because
LCS is a progressive disease.
