YEARS HUGE EXTRUDED DISCI C3-4 CENTRAL AND C4-5 RIGHT.
Anamnesis
The patient came to the clinic 09-November-2015
complaining of neck pain for 2 years with
exacerbation of severe agonizing neck pain and
cephalgia and pain shooting to both upper limbs
for 2 weeks. MRI done 31-October-2015 showing
huge central disc C3-4 and extruded disc C4-5
with compression of the spinal cord more to the
right.
On examination: the patient in agonizing pain
with limitation of neck movement to all
directions, more when turning the head to the
left and downward. There is weak biceps -4/5
both sides and the hand grip and flexion and
both triceps are -3/5. The patient is unable to
sleep for several days despite vigorous
medications.
Transnasal intubation was
difficult. Discectomy C3-4, C4-5 until the dura
was seen all over the posterior aspect.
Insertion of prefilled Samarys cervical cages
15x12x5 mm to C3-4 and C4-5 disc spaces. Fusion
of C3-4-5 using Trestle 28 mm cervical
cage with 5 screws fixed 4.5x16 mm and one
screw 4.5x16 mm to the lower left part. All
stages of surgery were performed with the C-arm.
Routine closure of the
wound.
Smooth postoperative recovery. The power of
both upper limbs became normal. The patient was
sent to the ward.
Comments
The patient in agonizing pain and with
severe neurologic impairment. Surgical treatment is a must
in this case to prevent more catastrophic sequels.
