12-AUGUST-2015  ISA IBRAHEEM HANASH  55 YEARS  LBP WITH LEFT SCIATICA DUE TO BULGE L3-4 AND L4-5.

 

Anamnesis

The patient came to the clinic 09-August-2015 complaining of LBP with left sciatica down to the dorsum of the left foot for 5 months. He was not limping with no scoliotic stance. SLRS was 80 degrees with pain left side. There is weak dorsiflexion left foot -4/5 and planterflexion same foot 4/5. MRI lumbar spine performed 25-January-2015 showed bulge L3-4 and L4-5.

The patient was sent for investigations and MRI repeated today showing the same bulges as before. Dynamic studies were negative for over mobility. It was suggested to the patient to undergo radiofrequency ablation using MultiGen and it was explained to him that the LBP can be treated but concerning the sciatica remain under question. He agreed.

Using the C-arm the left L2,3.4 and L5 points were localized and sensory test and motor test was applied for all points. The patient during sensory test claimed some desirable feeling. During motor test the canula was vibrating at 1.5 V. All 4 sites were ablated thermally using 80c and 90 sec. The patient was all the time interrogated and he is free of pain at the end of the procedure. He claims that there is still mild pain in the left SIJ.

The patient was discharged after 1 hour.

The patient came 3 days after the procedure telling that the LBP improved, but the sciatica to the left leg still the same. This mean that the RF ablation is capable to decrease the pain originating from the facet pathology. The major roots, which are not involved in the the facet changes, mostly will not be affected with such technology.

 

 

Comments

The patient has chronic LBP which is not responding to medication and surgical intervention is not needed. RF ablation was the best choice in this case.
