The patient  was complaining of severe LBP for more than 7 years with left sciatica and almost drop left foot for 5 years.  He had scoliotic stance for long time and hypalgesia of the left L5 and S1 dermatomes.  Left S1 foraminotomy and hemiflavotomy was performed and the extruded disc material was cleaned. There was actually no disc space to clean. Immediately after the surgery the power of the left foot improved, despite the fact it was a longstanding deficit.
