DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 28-October-2007
complaining of LBP for 4 months and right
sciatica for 1 month with numb right foot.
MRI performed 10-October-2007 showing huge
extruded disc L5-S1 with up and downward
migration right side.
On examination: the patient had exaggerated
scoliotic stance, limping with SLRS 30 degrees
in the right and 85 degrees in the left with the
pain shooting to the contralateral side.
Sensation for pin-brick was intact and she had
weak dorsi and planterflexion right foot.
Right L5-S1 hemiflavotomy with right S1
foraminotomy was performed and the extruded disc
was removed in one piece lateral to the axilla.
Meticulous cleaning of the disc space from the
right.
Routine closure of the wound and smooth
postoperative recovery with normalization of the
power of left foot.
Comments
The expected recurrence rate is around 7%
because the disc height change is still not noticeable and
the annulus fibrosis defect is not small.
