The patient came to the clinic 16-August-2002 complaining of LBP with right sciatica with weak dorsi and planterflexion right foot. He was operated elsewhere for PLD L2-3 10 years ago with little improvement.

On examination: The patient is in agonizing LBP with right sciatica with SLRS 45 degrees right side with pain. Weak dorsi and planterflexion right foot.

The patient was sent for investigations and MRI done showing still having huge extrusion L2-3 more to the right.

Refreshment of the wound and with slight extension up. During scarolysis a dural defect was seen at the upper border of the field and the root came up extradurally. It was left and covered by cotonoid and the patient was put in Trendelenburg position. The dura around the defect was expanded by drilling the bone circumferentially. Right L3 root neurolysis and removal of the disc of L2-3 lateral to the axilla. Intradiscal cleaning L2-3 from the right. After the dura and root became lax, the root was pushed back intradurally and the dural defect was closed by stitching by Nylon 4 zero. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Further inspection of the disc space from the right and the left side. Routine closure of the wound.

Smooth postoperative recovery. He was sent to the ward.

The patient came to the clinic 28-August-2002 with clean wound and dramatic improvement in his neurologic deficits.

There is still a possibility for disc re-recurrence around 7%, because the disc space is not shallow.
